African Health Initiative
Guide to Personal Care for You and Your Family

Providing Solutions to Health Care Challenges
Receiving the Care that You and Your Family Need. The African Health Initiative, also known as the African Health Net Project, is an innovative health care program of African Community International (The African Center, Indianapolis) that works with the African community of central Indiana to provide solutions to health care challenges. Funded by the Robert Wood Johnson Foundation and other central Indiana philanthropic foundations, the African Health Initiative’s main goal is to decrease the cultural, institutional, and personal barriers that prevent Africans and their families from accessing the health care system and creating sustainable systems of care. The program is available to all Africans who live within the state of Indiana.

The African Health Initiative’s first priority is on providing free, basic and preventative health care for Africans and their families. After enrolling in the program, Africans and their families will be able to see a health care professional. Just like a family or primary care physician, Africans can receive both treatment and prescriptions for a wide variety of medical conditions. Due to limited resources, the African Health Initiative cannot provide comprehensive long-term treatment or complicated medical procedures or treatment.

3 EASY STEPS Enrollment Process

There are 3 steps to enrolling in the program:

Step #1: Fill out the enrollment form provided by the African Center at address below
Step #2: Have your photos taken by us at the African Center
Step #3: Receive your African Health card in the mail after 10 days.

Now you are able to make an appointment with
our nurse practitioner and /or doctor.
